  • Completed by the head of station or office where the newly recruited member has assumed duty. It confirms date the member reported for duty at the given station.<\/li><\/ul>

    <\/path><\/svg><\/span><\/path><\/svg><\/span>ORIGINAL UNDETACHED MEDICAL EXAMINATION REPORT <\/span><\/path><\/svg><\/div>
    • Qualifies the member to be a contributor to the government pension scheme from the onset.<\/li>
    • If there are adverse comments on sections 78 to 83 of the medical examination form, SSB will photocopy, keep the copy and return the original medical examination form to the employing line Ministry so that they can refer the case to the Secretary for Health for his \/ her recommendations on whether to appoint the member as a pension contributor with restricted or with full benefits.<\/li><\/ul><\/div>\n <\/div>

      Duly completed and stamped by the Criminal Investigations Department (CID). This should be done before the person assumes duty.<\/p><\/div>\n <\/div>

      Either Bank Statement or written instruction from the newly recruited member to the Paymaster Salary Service Bureau\/ for his or her salary to be deposited into the member\u2019s preferred bank account.<\/p><\/div>\n <\/div>

      <\/path><\/svg><\/span><\/path><\/svg><\/span>CERTIFIED TRUE COPY OF THE ORIGINAL BIRTH CERTIFICATE <\/span><\/path><\/svg><\/div>
      • To determine the date of birth certificate, retirement date and if member qualify to be appointment on indefinite pensionable terms.<\/li><\/ul><\/div>\n <\/div>

        <\/path><\/svg><\/span><\/path><\/svg><\/span>CERTIFIED COPY OF IDENTIFICATION <\/span><\/path><\/svg><\/div>
        • National Identity card (I.D) to get the national identity number.

          A valid passport will also suffice.<\/p><\/li><\/ul><\/div>\n <\/div>
